Prospect Submission Form

Once this form is submitted online it will flow into the database. It should generate an email to all administrators. A form with check boxes should be generated, that once the boxes are checked the player will flow into the appropriate tables (Top Prospect, Top Pitcher, Top Catcher, etc.)
